determined that it was a mechanical failure , poor maintenance , and a lack of supervision from the General Direction of Civil Aerospace caused the helicopter crash that killed Puebla's governor Martha Erika Alonso, her husband Rafael Moreno Valle, and three other people last December 24, 2018 .

In President Andrés Manuel López Obrador's morning news conference in the National Palace, the head of the SCT Javier Jiménez Espriú revealed the final report on the accident in which different foreign agencies were involved, as well as the companies in charge of the construction of the aircraft and its engine.

According to the investigation, there is no evidence of a malfunction of the helicopter's essential components nor they detected any evidence of possible sabotage ; nevertheless, the linear warp actors of the Number Two stability system had already been repaired and there was previous knowledge of failures.
Jiménez Espriú said that on December 13, 2018 , according to the flight logbooks , it had already been reported that the linear warp action was damaged and needed to be replaced but the company kept operating the machine.

He stressed that a probable cause of the accident was a sudden warp to the left causing the helicopter to reverse its flight and crash.
The linear warp system had two loose screws that caused an unwanted movement, he said.
